About the Position

Job Title: RN Home Health Profession: RN Home Health Specialty: RN Home Health Duration: 13 weeks Shift: 08:00 AM - 04:30 PM Hours per Shift: 8 Experience: Minimum of 1 year of Certified Home Care experience, 1 year of Case Management experience License: RN License Certifications: N/A Must-Have: - 1 year Certified Home Care experience - 1 year Case Management experience Description: Applicants must have 1 year of Certified Home Care experience. 1 year of Case Management experience is also required. Experience in hospice, private care, or working as a Case Manager alone does not meet the qualifications for Certified Home Health Care Management experience. Only roles specifically within a certified home health agency will be considered relevant for the 1-year requirement. Ability to travel to assigned cases is necessary. Weekend rotations are required once every 6 weeks. As a Home Health RN, responsibilities include assessing patient conditions, implementing care plans, and providing ongoing education and support to patients and their families. Patient care is delivered within organizational philosophy, policy, and standards of community health practice. Admission processes and case management responsibilities must be performed in accordance with policies and standards of practice. Thorough and timely electronic documentation and interdisciplinary communications are essential.
