Care Manager

Position Overview Centene is looking for a Care Management Specialist to work with our 28 million members. This remote position is open to candidates residing in Missouri. Responsibilities Develop, assess, and facilitate complex care management activities for members with mental and behavioral health needs, providing personalized care plans and education for members and families. Evaluate member needs via phone or in-home visits, recommend or facilitate care plans/services for the best outcomes, addressing behavioral health and social determinant needs. Perform telephonic, digital, home, or other site visits to outreach and assess member needs, collaborating with resources. Develop ongoing care plans for high-acuity members, identifying providers, specialists, and community resources. Coordinate among members, caregivers, community resources, and care provider teams to ensure services are accessible. Monitor care plans, member status, and outcomes for changes in treatment side effects, complications, and clinical symptoms, providing recommendations based on identified needs. Facilitate care coordination and collaborate with providers or specialists to ensure timely access to needed care or services. Collect, document, and maintain member information and care management activities to ensure compliance with state, federal, and third‑party payer regulations. Provide education to members and families on procedures, provider instructions, treatment options, referrals, and benefits, including behavioral health and social determinant needs. Provide feedback to leadership on opportunities to improve and enhance care quality and delivery for members cost‑effectively. Perform other duties as assigned and comply with all policies and standards. Qualifications & Requirements Education: A Master’s degree in Behavioral Health or Social Work, or a degree from an accredited School of Nursing, and 2–4 years of related experience. License/Certification: Licensed Master’s Behavioral Health Professional (e.g., LCSW, LMSW, LMFT, LMHC, LPC) or RN, based on state contract requirements, with behavioral health experience. Compensation: $56,200 – $101,000 per year. Actual pay will be adjusted based on skills, experience, education, and other job‑related factors.
