Job Summary The Manager for Case Management Services reports to the Case Management Director and is responsible for promoting patient‑centered care by coordinating all aspects of hospital‑based case management for the assigned area of responsibility in alignment with the goals of the Case Management Department. The Manager oversees day‑to‑day operations, coordinates, supervises, and provides administrative oversight of the case management team, ensuring effective care coordination and practices that comply with hospital policies, applicable regulations, and guidelines. The Manager monitors quality of interactions, documentation, and assessments of the staff, and coordinates activities that promote quality outcomes, patient throughput, optimal care, and appropriate resource utilization.
