Physical Therapist

The Physical Therapist is responsible for evaluating, planning, and implementing individualized treatment programs for patients referred for physical therapy services. This role focuses on helping patients achieve optimal functional outcomes through evidence-based interventions, patient and family education, and close collaboration with an interdisciplinary team. Responsibilities/ Job Duties: Creates and maintains detailed documentation of patient evaluations, treatment plans, and progress updates. Conducts thorough assessments upon receiving physician referrals and establishes practical, patient-specific goals in coordination with the patient, their family, and the referring provider. Designs and carries out therapeutic interventions tailored to individual needs, ensuring safety measures are followed. Educates patients and their families on therapy techniques, home exercises, and lifestyle adjustments to support ongoing progress. Works collaboratively with a multidisciplinary team—including doctors, social workers, nutritionists, vendors, and other staff—to address each patient’s rehabilitation and equipment needs. Engages in quality improvement activities to enhance care delivery and patient outcomes. Participates in departmental meetings, clinic assignments, and patient rounds as required. Minimum Education and Experience Qualification Requirements: Active physical therapy license in the applicable state. Degree from an accredited physical therapy program.
