Remote Senior Clinic Pharmacist Chronic Care Leader

CenterWell Primary Care is seeking a Clinical Pharmacist to join our remote care team. You will use evidence-based medicine and clinical judgment to optimize pharmacotherapy for high-risk patients, collaborate with physicians and nurses, and support CMS Stars and HEDIS initiatives. You will manage post-discharge medication reconciliation, provide patient counseling, stay current with guidelines, and document all activities in the EMR while delivering quality care across markets within a remote #J-18808-Ljbffr
