Function The Physical Therapist will carry out an individualized program of physical therapy including assessment, treatment, planning, implementation, education, and communication to maximize a patient's progress toward achieving functional goals. Responsibilities Provide direct patient care, evaluate functional needs and outcomes, and consult with other specialists. Review patients’ functional needs and adjust care plans to ensure optimal patient outcomes. Design and implement a plan of care for patients based on thorough assessment and incorporate participation by significant others when appropriate. Conduct therapy practice within defined standards of care. Direct the assistance of support staff to achieve patients’ functional goals. Communicate effectively with other professional and support staff to achieve positive patient outcomes. Practice hospital and departmental policies and procedures, objectives, quality improvement program, safety, environmental, and infection control standards. Document evaluations, treatment goals, and plans; regularly update documentation and maintain prescriptions and signatures. Enhance professional growth and development through participation in educational programs, in-service meetings, and workshops. Keep up to date on professional literature. Provide direction and leadership to staff and act as a resource to promote physical therapy in the community. Read, write, and speak English sufficient to fulfill job requirements. Make independent decisions, solve practical problems and interpret instructions as needed. Listen actively and evaluate social situations objectively, consistently and fairly. Comply with community policies and procedures for workplace safety, infection control, emergency response and safety protocols. Comply with all company policies related to resident rights, abuse prevention, reporting, and confidentiality (HIPAA). Demonstrate knowledge of Employee Handbook and comply with its policies. Perform other related duties as assigned. Qualifications Knowledge and skills to care for the physical and developmental needs of the population age groups receiving services. Ability to remain calm in emergency or crisis situations. Graduate of an American Physical Therapy Association (APTA) approved physical therapy program. At least one year of experience as a physical therapy aide. Current license from the state Board of Physical Therapy Examiners.
