Vincentian Home Personal Care in McCandless, PA is looking for a compassionate and experienced Memory Care Coordinator to provide leadership and oversight for our Secured Dementia Care Unit. This position plays an important role in creating a safe, supportive, and engaging environment for residents living with dementia while providing guidance and support to the team members who care for them.

The Memory Care Coordinator will serve as a knowledgeable resource and visible leader on the unit, helping ensure that residents receive individualized, person-centered care that promotes dignity, independence, engagement, and quality of life. This position works closely with the Personal Care Home Administrator and interdisciplinary team to coordinate daily operations, support staff development, communicate with families, and ensure resident needs are appropriately assessed, documented, and addressed in accordance with Department of Human Services (DHS) requirements.

What You'll Do

• Provide daily leadership, oversight, and coordination for the Secured Dementia Care Unit, promoting resident safety, dignity, engagement, and quality of life.
• Model compassionate, person-centered dementia care practices and support staff in responding effectively to residents' individual needs, preferences, and behaviors.
• Observe and support the daily work of unit staff, promoting accountability, teamwork, compassionate communication, and consistent dementia-capable care.
• Coordinate and provide dementia-focused education and training, including required DHS dementia training and ongoing coaching related to communication, behavioral approaches, de-escalation, engagement, and family support.
• Monitor resident needs and changes in condition, support implementation of care plans, and communicate concerns or changes to appropriate members of the care team.
• Maintain and update Resident Assessment and Support Plans as needed to ensure resident needs, preferences, strengths, risks, and supports are accurately documented and followed.
• Build positive relationships with residents, families, and responsible parties by providing compassionate communication, addressing questions and concerns, supporting transitions, and fostering trust and partnership.
• Collaborate with the Personal Care Home Administrator, nursing, personal care, activities, dining, therapy, and other interdisciplinary team members to promote coordinated care and effective unit operations.
• Incorporate the eight dimensions of wellness-emotional, environmental, financial, social, physical, spiritual, occupational, and intellectual-into resident experiences and engagement opportunities.
• Promote an environment in which residents, families, visitors, and team members are treated with kindness, respect, and dignity.
• Perform other duties as assigned by management.

What We're Looking For

• Bachelor's degree in nursing, social work, gerontology, health care administration, human services, or a related field preferred. An equivalent combination of education, certification, training, and experience may be considered.
• Experience in dementia care, memory care, personal care, assisted living, long-term care, or a related healthcare setting required.
• Previous supervisory, leadership, coaching, or staff training experience preferred.
• Strong communication, organization, problem-solving, and interpersonal skills.
• A compassionate, resident-focused approach and a genuine commitment to supporting individuals living with dementia.
