Mental Health Therapist Internship - Chicago - River North

General Duties Perform the following responsibilities for all mental health clients: Schedule and complete an initial Mental Health Assessment reviewed and approved by an LPHA. Complete the Level Of Care Utilization System (LOCUS) functional assessment for clients 18 years of age and older. Document an Admission Note summarizing identifying information, presenting problem, history of present problem, provisional diagnosis, safety precautions, medical necessity of treatment, and treatment plan recommendations. Re-assess per Medicaid Title 132 guidelines for ongoing patients. Mental Health Assessment Schedule and complete an initial Mental Health Assessment; approval by LPHA required. Complete LOCUS assessment for clients 18 years and older. Document Admission Note including identifying information, presenting problem, history of present problem, provisional diagnosis, safety precautions, medical necessity of treatment, and treatment plan recommendations. Re-assess per Medicaid Title 132 guidelines for ongoing patients. Treatment Planning Create and document a timely individualized treatment plan with client (and guardian if applicable) addressing identified needs; review and approval by LPHA. Update treatment plan promptly as client progresses. Mental Health Therapy and Case Management Provide individual, group, couples/family counseling as needed per assessment and treatment plan. Maintain progress notes documenting goals, interventions, client response, and continued therapy plan. Offer mental health case management to link and coordinate needed services in the community. Intervene in crisis situations, coordinating appropriate services. Work Schedule Determined by Educational Institution. Work Location 310 W.
