Burlington Health & Rehabilitation Center is seeking a compassionate and dedicated Speech Therapist (Speech-Language Pathologist) to join our therapy team. This is an excellent opportunity to provide quality rehabilitation services in a supportive, collaborative environment focused on resident-centered care. $7,500 Sign-On Bonus Now Available for Full-Time Employees! Responsibilities Provide speech therapy services that promote excellence in rehabilitation care and quality patient outcomes. Promote interdisciplinary cooperation and support the facility's geriatric rehabilitation program. Evaluate patients in a timely manner according to state, payer, and facility requirements. Screen patients upon admission and/or when requested by the Director. Develop individualized treatment plans and appropriate goals based on patient evaluations. Provide treatment according to the established plan of care. Communicate and collaborate with other disciplines and staff as indicated by the treatment plan. Provide routine documentation in accordance with Medicare, facility, and other payer guidelines. Participate in family conferences when requested. Record and submit all required documentation and treatment charges in a timely manner according to facility policies and procedures. Qualifications Current Vermont State License as a Speech-Language Pathologist. Ability to work effectively as part of a dynamic and cooperative interdisciplinary team. Strong assessment and evaluation skills. Ability to develop individualized treatment plans and appropriate discharge plans. Ability to participate in consultations, teaching, and meetings with residents, families, staff, and other healthcare professionals. Strong communication and interpersonal skills. Ability to educate and counsel patients, families, and staff as appropriate.
