Physical Therapist - Travel Contract

Looking for a change of scenery and a chance to help others. Join a traveling Physical Therapist team providing care in Moore, OK at a skilled nursing facility. You will screen, evaluate, and rehabilitate patients with physical functioning disorders and wound care, and develop appropriate plans of care following regulatory and clinical practice standards. Utilize standardized assessments and evidence-based practice to support clinical interventions, including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care. Document treatment minutes and clinical records per regulatory requirements. Communicate patient progress with supervisors and interdisciplinary team members. Participate in patient care conferences, utilization review, and required facility trainings. Supervise or support physical therapist assistants, rehabilitation aides, and students as needed.

Minimum Qualifications

• Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active Oklahoma physical therapy license required to start the assignment.
• Minimum 1 year of PT experience required. New graduates acceptable only with Level II clinical experience beyond Level I observation.
• Experience in long term care or skilled nursing/rehabilitation settings required.
• BLS certification from AHA or ARC may be required.
• Ability to document in electronic medical records and meet regulatory documentation requirements.
• Ability to obtain necessary DME or supplies per regulatory and clinical practice requirements.

Preferred Qualifications

• Experience with wound care interventions.
• Experience supervising PTAs, aides, or students.

Assignment Details

• Shift: 8 hour days, 35 GWW.
• Weekend requirement: None.
• May be required to float to other facilities within the region as needed.
