RN Field Care Navigator

Molina Healthcare in Elgin, IL is seeking an experienced Care Manager RN to support care management and care coordination across the care continuum. You will work with a multidisciplinary team to advance member outcomes while improving quality and cost efficiency. Responsibilities include comprehensive assessments, developing and implementing collaborative care plans, and conducting visits as needed.
