Rate of Pay: $27.00 to $30.00 Per Hour Depending on ExperienceMHM Services, a Centurion company, is proud to be the provider of mental health services to the California Department of State Hospitals-Forensic Assertive Community Treatment Program (FACT). The FACT program is a statewide system that provides an unlocked, supervised living environment with 24-hour staffing. FACT's model of care is centralized services with a slow transition to community-based services. Services needed to support community reintegration include but are not limited to the following:Forensic mental health treatment through individual and group therapy settingsLife skills trainingDay socialization groupsCollateral contactsHome visitsSubstance abuse screeningsPsychiatric servicesCase ManagementCourt reportsPsychological assessmentsSchedule: Monday through Friday; 8:00am to 4:30pmUnder the direction of the Program Director the Substance Abuse Counselor is responsible for providing support, guidance, and mentoring the patients in a community setting to meet service delivery requirements of the FACT Program.Essential Duties/ResponsibilitiesProvide intake, assessment, treatment planning, group and individual counseling to clients in the Substance Use Disorders/Addiction programProvide after-care plan for clients returning to the community including program referrals to substance use disorders/addiction treatmentMaintain accurate and thorough clinical records per Company and DSH requirements in a timely mannerProvide input and reports to the Clinical Supervisor regarding the clinical needs of clients and programming, improving ways of offering prevention, outreach, and treatment servicesWork collaboratively with behavioral/mental health and medical staff when clients have co-morbid conditionsCommunicate with treatment and case management staff through daily briefing and debriefing between shiftsAttend required staff meetings, safety training and in-service programs as requiredIdentify and maintain professional boundaries to preserve the integrity of treatment and the professional relationshipProactive in performance improvement activities as demonstrated by participating in the Continuous QualityHM/Centurion offers a comprehensive benefits package for our full-time team members including:Health, dental, vision, disability and life insurance401(k) with company matchGenerous paid time offPaid holidaysFlexible Spending AccountContinuing Education benefitsMuch more...QualificationsCertified Alcohol and Drug Counselor (CADC) certification preferred, but will accept other Alcohol and Drug Counseling CertificationsMust be able to obtain CPR certificationMust be able to pass background investigation and obtain agency security clearance where applicableValid State Driver’s Licenseindmhm #J-18808-Ljbffr Centurion Health
