This position focuses on high-risk member assessments and care coordination within a dedicated team of healthcare professionals. Key responsibilities of the RN New Hanover include:
- Perform comprehensive assessments of high-risk members
- Collaborate with primary care providers to implement individualized care plans

Requirements of the RN New Hanover include:
- Associates or Bachelor's Degree in Nursing or related field
- Current, unrestricted state Registered Nurse license
- Ability to analyze complex medical information and make rational decisions
- Case Management certification
