Clinical Transition Specialist RN

Onsite | South Bend, Indiana

Position Summary

We are seeking a compassionate and relationship-driven Registered Nurse (RN) to join our team as a Clinical Transition Specialist RN in Farmington, MI. This onsite role focuses on supporting patients transitioning from inpatient care to home infusion and alternate site services.

The ideal candidate combines strong clinical knowledge with exceptional communication and relationship-building skills. This role acts as both a clinical liaison and healthcare sales professional, helping educate patients and referral sources while ensuring smooth and coordinated discharge planning.

Key Responsibilities

• Transition patients from inpatient settings to home or alternate site care services

• Promote home infusion and related services to discharging patients and healthcare partners

• Educate patients, families, and facility staff regarding home infusion therapies and available support services

• Assess patients for eligibility and appropriateness for home-based or alternate site care

• Coordinate care transitions and collaborate with discharge planners, case managers, physicians, and interdisciplinary teams

• Build and maintain strong referral relationships with hospitals, providers, and healthcare facilities

• Partner with Account Managers to increase referral volume and support business growth initiatives

• Review medical records and collect pre-admission, clinical, and insurance information

• Facilitate communication across care teams to ensure timely treatment initiation and continuity of care

• Support patient education and encourage therapy independence and adherence

• Coordinate logistics related to supplies, orders, scheduling, and care setup

Qualifications

• Active Registered Nurse (RN) license

• Previous experience in care coordination, case management, discharge planning, infusion nursing, home health, acute care, or transitional care preferred

• Strong interpersonal, communication, and relationship-building skills

• Comfortable working in a patient-facing and referral-development environment

• Ability to review clinical documentation and coordinate multidisciplinary care plans

• Experience with insurance verification or healthcare authorization processes is a plus

• Proficient with electronic medical records and clinical documentation systems

Preferred Experience

• Home infusion or specialty pharmacy experience

• Hospital discharge planning or transitional care experience

• Healthcare sales or clinical liaison background

• Experience collaborating with physicians, case managers, and referral sources
