Typical Work Schedule: Casual PRN or 12 hour shift

Day Shift: 6am to 6pm
Night Shift: 6pm to 6am

Casual Shifts: Casual/PRN employees work on an ‘as needed’ basis to meet the needs of the business.  Work days/hours will often vary from week to week.

Execution of the prescribed diagnostic and therapeutic treatment regiments, administering, delegating, and evaluating nursing activities.

Responsibilities:

Essential Functions

1. Provides an accurate and thorough assessment of the patient/family to identify human responses to actual or potential health conditions.

• Performs patient care assessments and reassessments based on patient condition and established policy/procedure.
• Demonstrates the ability to effectively utilize the nursing process when providing assessment for all individuals.
• Demonstrates the ability to assess and reassess pain and pain management techniques utilized.
• Documents all assessment and reassessments in an accurate, legible, and timely manner.

1. Formulation of a nursing diagnosis to identify and plan for the needs of an individual, family, or group.

• Utilizes history and assessment information to identify patient problems and set goals.
• Formulates a teaching plan based upon identified learning needs.
• Demonstrates the ability to revise a plan of care as indicated by patient's response to treatment.
• Utilizes assessment data and formulates a plan for discharge needs.

1. Implementation of a nursing treatment regimen to provide for the selection, performance and management of proper nursing practices.

• Performs patient care responsibilities considering needs specific to the standards of care and patient's age.
• Performs direct patient care utilizing professional nursing skills and judgment according to physicians and other licensed practitioner's orders.
• Teaches health care practices, initiates and participates in patient and family education programs.
• Demonstrates knowledge of medications and their correct administration based on age of the patient and their condition.
• Documentation of nursing care meets current standards and policies.

1. Evaluation of the plan of care an treatment for all patients under his/her care to promote resolution of patient problems and positive patient outcomes.

• Evaluates overall plan daily for effectiveness and makes adjustments as needed to meet patient goals.
• Evaluates effectiveness of learning and discharge teaching needs.

1. Coordinates and supervises patient care to provide for the management of a multidisciplinary approach to patient care.

• Communicates pertinent information regarding patient care appropriately and clearly.
• Demonstrates the ability to assist physicians with procedures and patient rounds.
• Utilizes effective clinical delegation skills, demonstrating accountability for the scope of practice of the individual being delegated to and evaluation of the outcome of delegation.
• Consults with co-workers and other departments as appropriate to provide for an interdisciplinary approach to the patient's needs and discharge plan.

1. Safety initiative - Complies with Methodist Fremont Health safety policies and procedures and regulatory requirements e.g., Occupational Safety and Health Administration (OSHA), Joint Commission (JCAHO), Clinical Laboratory Improvement Amendments (CLIA).

• Performs job duties in a manner consistent with organization and departmental safety policies and procedures. C
• Completes departmental safety requirements, I.e., blitz, departmental in-services, etc., on an annual basis.

1. For Home Health: Understand and implement Medicare requirements in care of the home care patients to meet eligibility, documentation and compliance.

• Patient care is planned and provided based on a comprehensive assessment of patient needs.
• Utilizes the role of primary nurse to effectively manage patient care from admission to discharge in an appropriate and cost effective manner.
• Documentation is completed in a timely manner and supports the care provided to meet regulatory requirements.
• Utilizes data from the Oasis comprehensive assessment to plan patient cares and monitors outcomes of the care.

1. For Home Health: Coordinates care of home care patients to provide for Continuity of Cares.

• Coordinates care of home care patients to provide for Continuity of Cares.
• Works as an integral part of the Hospice Interdisciplinary team to meet the ongoing needs of hospice patients.

Job Requirements

Education

• Graduate from an accredited Diploma School of Nursing/College of Nursing with an Associate of Science Degree in Nursing (ASN) or Bachelor of Science in Nursing (BSN) required.
• Bachelor of Science in Nursing (BSN) preferred.

License/Certifications

• Required:
  • Current valid Nebraska Registered Nurse (RN) License, valid compact multistate license, or a temporary permit while awaiting licensure required.
  • Current American Heart Association or American Red Cross Basic Life Support (BLS) strongly preferred at time of hire, required within 3 months of hire.
• Preferred:
  • 4th LDRP/Peds: Electronic Fetal Monitoring Certificate (C.EFM) of Added Qualification from NCC within 1 year of eligibility.

Experience

• Two to five years previous inpatient nursing experience preferred.

• Knowledge of computer resources.
