Description PRN Physical Therapist - Flexible Schedule - You don't want full time work? This is for you! Work when it works for you. We are seeking a PRN Physical Therapist who is passionate about delivering exceptional, one-on-one patient care and contributing to a supportive, team-focused environment. This role offers flexibility and the opportunity to support our Schaumburg clinic and surrounding Northwest suburban locations.

- Develop and implement individualized plans of care based on evaluation findings

- Deliver hands-on, one-on-one therapy to optimize patient outcomes
- Monitor patient progress and communicate effectively with referring providers and care teams
- Educate patients through customized home exercise programs
- Manage discharge planning and ensure continuity of care
- Maintain compliance with payer and documentation standards
- Stay current with best practices through continuing education and professional development
- Collaborate with healthcare providers, case managers, and insurance partners
