Position Summary Under the leadership of the Center Manager, the Home Care Coordinator is responsible for planning, monitoring, and scheduling in‑home services and works closely with the Personal Care Supervisor (PCS) to ensure all personal care needs of participants are met. Essential Functions Ensure home care schedules are maintained accurately and participants are allocated sufficient time/services. Coordinate care through staffing with contracted home care providers and Personal Care Attendants (internal staff). Coordinate visits, considering the qualifications of the Personal Care Attendant and participants’ level of care. Re‑assign daily visits if a staff member can’t conduct visits as scheduled; inform PCS when visits cannot be rescheduled. Proactively modify schedule to ensure accuracy; notify participant and/or primary caregiver in advance of changes. Participate as a member of the Interdisciplinary Team (IDT) in initial, semi‑annual, unscheduled, and annual assessments. Attend morning updates and report changes in participants’ status to appropriate staff regularly. Perform home‑care assessments for Service Delivery Requests (SDR) and update EMR per regulatory guidelines. Collaborate with PCS and IDT, and may participate in initial enrollment meetings with families and others. Submit authorization requests to home‑care agencies as required. Fill in as PCA when needed or in emergent situations. Assure documentation for home care is accurate, current, and entered in EMR timely. Maintain confidentiality regarding participant, staff, contractor, and organizational information. Non‑Essential Functions Attend and participate in staff meetings, in‑services, projects, and committees as assigned. Adhere to and support the center’s policies, practices, and procedures. Accept assigned duties cooperatively and perform all other related duties as assigned. Be flexible in schedule of hours worked. Require use of personal vehicle. Education and Certification High school diploma or GED required. LVN/LPN preferred. Current driver license. Proof of auto insurance. Clean driving record. BLS and CPR certification. Skills and Experience Minimum two years of demonstrated successful experience in home care, preferably in‑home care management. Minimum one year of documented experience working with frail or elderly population. Experience in providing person‑centered care, including assessment, personal care, and health education. Working knowledge of physical, mental, spiritual, and social needs of frail elderly and their families. Effective time‑management skills. Knowledge of utilization review, quality improvement, and cost containment systems. Ability to work with ethnically diverse populations. Creative problem‑solving skills to address complex home‑care issues. Collaborate with interdisciplinary teams. Strong organizational, flexible, resourceful, and energetic. Effective oral and written communication skills. Medical Clearance Employees must have medical clearance for communicable diseases and up‑to‑date immunizations before direct participant contact.
