More than 2,500 team members at more than 70 Tidelands Health locations bring our healing mission to life each day.A Brief Overview Responsible for providing a full range of therapeutic services including assessment, treatment planning and therapeutic interventions in an interdisciplinary environment consistent with the position's qualification, professional practices, and ethical standards. A physical therapist (PT) must be able to evaluate a patient's condition and devise a customized physical rehabilitation and treatment plan. The PTs' goal is to develop, maintain and restore maximum movement and functional ability, to maximize quality of life in an outpatient rehab setting. The physical therapist shall also demonstrate accountability for the contribution to program development, quality improvement, problem solving, and productivity enhancement in a flexible interdisciplinary fashion. The employee may have job functions where they will handle and deliver medications based on the requirements of their job in the assigned work facility. The employee will complete other duties as assigned.What you will doReview physician's referral (prescription), patient's condition, medical history, and submitted medical records to determine physical therapy treatment required. Plans and prepares written treatment program based on evaluation of patient and incorporate proper interventions that can help patients better perform everyday tasks, decrease pain, ambulation, and improve strength and mobilityEvaluate patients' functional ability through various standardized tests, measures patient's strength, motor development, sensory perception, functional capacity, and level of independence with daily activities, and records findings to develop treatment programsDevelop a treatment plan for patients with specific goals and tasksAdminister treatments involving application of physical agents, such as cold packs, contrast baths, moist packs, ultraviolet and infrared lamps, Iontophoresis, electrical stimulation, and ultrasound machinesEvaluates effects of treatment at various stages and adjusts treatments to achieve maximum benefitRecord treatment, responses, and progress in the patient's electronic medical record and prepare reports as indicated by licensure, the provider, and or insurance recommendationsInstruct patient and family in treatment procedures to be continued at homeComplete documentation per clinical / department guidelines and meets expectations of monthly department chart auditsPromote/educate Organizations Rehabilitation programs to referring physicians, healthcare providers, and or co-workers by performing in-services and/or marketing activities.Reduce cancel/no-show rates to improve quality of care and patient outcomesAchieve the established average monthly patient care productivity metricEducation QualificationsBachelor's Degree RequiredMaster's Degree if obtained prior to 2016 PreferredDoctoral Degree if obtained after 2016 RequiredSkills and AbilitiesAbility to utilize therapy equipment in an outpatient rehabilitation setting to provide treatment and proceduresPunctual and dependable for assigned/confirmed shiftsAbility to respect cultural and religious practices of patientsLicenses and CertificationsPhysical Therapist - South Carolina Department of Labor, Licensing and Regulation RequiredBasic Life Support Certified - American Heart Association within 30 Days RequiredPhysical Demand Medium-Heavy Physical DemandThe intent of this job description is to provide a representative and level of the types of duties and responsibilities that will be required of positions given this title and shall not be construed as a contract for employment nor a declaration of the total of the specific duties and responsibilities of any particular position.
