Under general supervision, the Community Health Worker (CHW) is a frontline public health worker who performs a wide variety of duties at the paraprofessional level to engage, support, advocate and educate Southern Maryland community members in overall health and wellness. The CHW serves as a liaison or link between health and social services and the Charles County community to facilitate access and improve health outcomes by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, social support and advocacy. Qualified candidates must have a GED or high school diploma and (3-5) years of experience in the health/social services field. The candidate is preferred to have an Associate Degree in a health/social services related field. SKILLS & ABILITIES Knowledge of local population demographics, assets and needs Knowledge of local community health disparities such as diabetes and hypertension Ability to connect with and provide ongoing support in a behavior change process Demonstrated organizational and time management skills Effectively communicate in writing in order to document patient information and progress Ability to follow instructions and incorporate feedback Communicate verbally with patients and co-workers in a clear and effective manner Able to work independently and as part of a multidisciplinary team Knowledge and skills to screen high risk patients for post discharge needs and ensure the effective communication is established with the patients, caregivers, families, and all disciplines to ensure a successful transition across the continuum of care The ability to ensure the confidentiality and rights of all patient and departmental documents is required Benefits All your information will be kept confidential according to EEO guidelines.
