A Long-Term Acute Care (LTAC) hospital with two locations in Marin County is seeking a Pharmacist to join their Kentfield, CA site. This opportunity offers a flexible schedule, with part-time hours and the potential to expand to full-time. The role focuses on providing clinical pharmacy services within a 50-60 bed facility and leverages systems such as Wellsky and PYXIS automation.

Responsibilities:

• Manage medication dispensing and distribution using PYXIS automated systems.
• Provide clinical pharmaceutical care in an inpatient LTAC setting.
• Collaborate with healthcare teams to optimize pharmacokinetics, anticoagulation, and TPN therapies.
• Maintain compliance with hospital policies and regulatory standards.
• Support clinical decision-making to improve patient outcomes.

Qualifications and Desired Experience:

• Bachelor of Science degree from an accredited College or School of Pharmacy required; Doctor of Pharmacy degree preferred.
• Valid California Pharmacist License required.
• 2-3 years of retail or hospital pharmacy experience preferred, especially candidates maintaining clinical expertise.
• Inpatient or acute care experience is highly advantageous.
• Familiarity with pharmacokinetics, anticoagulation, and Total Parenteral Nutrition (TPN) preferred.
• Current, valid BLS and ACLS certifications from approved providers preferred.
• Candidates with extensive community pharmacy experience (10+ years) are not preferred.
• PGY1 residency experience recognized; however, candidates seeking long-term positions at small facilities are advised to consider fit.
