Recovery Support Navigator

Discounts on wide array of services/entertainment nationwide The Recovery Support Navigator Is Responsible For Providing non-clinical services to engage, educate and support patients seeking treatment upon release and their families in order to motivate and maintain them in culturally relevant behavioral health services in the community Beginning pre-release, and the support continues after the patient is released and returns to the community Managing an assigned caseload and completing all required documentation for services as required by Spectrum, DOC and all licensing, accreditation and regulatory bodies within established timeframes Discharge planning that addresses initiation and/or continuation of SUDT services, with or without medication for addiction treatment Coordinating a re-entry package for each patient scheduled for release, and documents receipt of nasal naloxone Qualifications: High school diploma required and LADC-II preferred Bachelor's degree in human services or related field or equivalent experience in the behavioral health field preferred Case management experience strongly preferred #J-18808-Ljbffr Spectrum Health Systems, Inc.
