Established in 1912, the Jewish Home of CNY evolved from a small rooming house in downtown Syracuse to become the 36-acre campus in DeWitt known as Menorah Park. If you're looking to be a part of a friendly team that delivers quality care in a safe, quiet, neighborhood setting, Menorah Park might be the right place for you.Rn Supervisor for 132 Bed Facility Long Term Care, Skilled Nursing Facility.BROAD RESPONSIBILITIES:Involved in admissions and assessments, play a crucial role in patient care by gathering essential information to inform treatment plans, conduct thorough assessments, verifying patient information, and developing individualized care plans based on patients needs. Patient Assessment: Performs comprehensive evaluations, including physical assessments and health histories, to determine patient needs.Medication Reconciliation: Ensure accurate medication list are maintained, identifying any discrepancies.Care Plan Development: Accurate documentation of patient information and care plans is essential for continuity of care.Patient Education: Explain facility policies and procedures to residence and families, addressing any concerns.
