Care Navigator
Community Reach Center
Community Reach Center seeks a Care Navigator to provide navigation and support services within its Aware Division. You will work with consumers to connect to behavioral health resources, using a case management approach and coordinating with various community supports. The role involves documentation in the EHR (Credible), coordinating referrals, and partnering with a team to ensure timely and confidential service delivery.
