Triage Visit RN - Home Care

Triage Visit Registered Nurse - Home Care Hours Part‑time, weekend days. Available every other weekend to take calls, triage patients, and make visits as needed. Remote work with optional visits. General hours: 8:00 am – 5:00 pm Saturday and Sunday. Training Availability during weekdays (Monday‑Friday, 8:00 am‑5:00 pm) for a minimum of 2 days per week, approximately 2‑6 weeks, depending on availability. Responsibilities Take calls from the On‑Call Service and address them based on type of calls (e.g., Intake/Referrals, Scheduling, Patient Concern). Provide telephone triage and make patient visits, as appropriate. Complete accurate and timely documentation of patient/family services provided. Provide education and instructions to the patient/family in accordance with the plan of care. Administer medications and treatments as prescribed by the physician and in accordance with nursing standards of practice. Benefits Hourly pay range: $38.46 – $46.86 Mileage reimbursement 401(k) Scrubs allowance Recognition programs Locally owned and operated Qualifications Graduate of an accredited school of nursing and licensed to practice as an RN in the state of MN. License is active and in good standing. Minimum of twelve (12) months of accumulated experience as a registered nurse within the last five (5) years. Able to travel within a geographic area serviced by the office. Valid driver’s license, reliable transportation, and proof of auto insurance required. Current CPR certification required.
