Physical Therapist Assistant - Home Care

Overview Provides physical therapy interventions to patients under the supervision of a licensed Physical Therapist (PT). Assists in monitoring patient progress, documents care, and follows established plans of care. Works within the scope of a Physical Therapist Assistant (PTA), including co-visits with PTs and patient re-assessment every 6 visits or 30 days. Manages a caseload, delivering services at patients' homes or facilities, and assists with therapeutic activities, transfers, and use of medical equipment. Ensures care complies with agency policies and standards, utilizing technology and infection control materials during visits. Participates in team meetings and special projects. Must be able to travel and handle various environmental conditions. Qualifications NY State Certification as a Physical Therapist Assistant required Associate's Degree from an approved program or equivalent Minimum of two years clinical patient care experience Valid driver's license may be required Bilingual skills and basic PC skills preferred #J-18808-Ljbffr
