Population Health Program Manager

To support quality improvement initiatives, the full-time remote Population Health Program Manager will identify, implement, and guide projects related to CMS, HEDIS, and health plan initiatives while serving as a liaison between Hoag Clinic and various enterprise teams. Key responsibilities Oversee the planning and integration of technology and systems for performance initiatives Act as a liaison between Hoag Clinic and IT, Cybersecurity, Compliance, and Marketing teams Participate in continuous improvement processes to enhance the quality of care and patient experience Required qualifications 3 to 5 years of experience with application support of physician practice systems Bachelor of Science degree in Computer Science, Business, Healthcare, or related field Experience with CMS, HEDIS, and Quality Improvement programs
