Job Summary:

Responsibilities:

• 
  Administer medications and treatments as prescribed, monitoring patient responses.

  Perform clinical assessments and document patient progress accurately.

  Assist with activities of daily living, including hygiene, nutrition, and mobility support.
  
  
• 
  Collaborate with healthcare team members to develop and implement care plans.

  Maintain strict adherence to safety protocols and infection control policies.

  Ensure documentation and record-keeping comply with regulatory standards.
  
  

Qualifications:

• 
  Hold a valid state license as an LVN or LPN.
  
  
• 
  Bring a current COVID-19 vaccination or valid exemption documentation.

  Possess strong clinical assessment and patient care skills.

  Demonstrate excellent communication and teamwork abilities.
