The LPN is prepared to function as a healthcare team member by exercising sound nursing judgment based on preparation, knowledge, experience in nursing, and competency. Appropriately and respectfully delegates to other team members.Patient Evaluation and Care Knowledge of disorders and injuries of patients and the ability to perform an evaluation and follow and contribute to a treatment plan/plan of care in partnership with the care team.Patient and Designated Care Partner Education Reinforcement Knowledge of child and adult education techniques and the ability to elicit learning priorities and needs. The LPN is responsible for providing follow up education to patients and their designated care partners on the patient's condition, the treatment and medications suggested and given, and the next steps in the patient's recovery.Medication Administration and Prescription Knowledge of various medications used to treat patients, ability to apply this knowledge to the administration of medications as allowed by the licensed practical nurse scope of practice defined by the state of practice Nurse Practice ActsDischarge Planning Knowledge of the patient's post-discharge needs and the ability to partner with the patient and family/designated care partner(s) in reinforcing a discharge plan and arranging for follow-up care.Evaluation and Diagnosis Knowledge of various tools and techniques for patient observation, evaluation and nursing diagnosis or clinical judgement, ability to explore and analyze reasons behind symptoms and generate resolutions according to available drugs, facilities and techniques as allowed by the licensed practical nurse scope of practice defined by the state of practice Nurse Practice ActsHealth Information Documentation Knowledge of the relevance of health information and appropriate documentation methods, ability to record medical histories, symptoms, vital signs, medications, diagnoses, treatment plans, procedures, tests, and medical results in written or electronic form.BLS - Basic Life Support - American Heart AssociationLPN - Licensed Practical Nurse_PA - State of Pennsylvania orLPN - Licensed Practical Nurse_NJ - State of New JerseyPhysical Demands Lift and carry 40-50 lbs. Examples: Push/pull patients on bed, stretcher (requires 29 lbs. push force), lateral transfers up to 50 lbs. of the patient's weight. Frequent to continuous standing/walking. Patient transporters can walk 8-10 miles per shift. *Patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.Salary Range$27.58 to $39.30 HourlyThe actual hiring rate will be determined based on candidate experience, skills and qualifications.
