$120,000 – $150,000 yearly DOE + Bonus Opportunities

Responsibilities:

• Lead and inspire a team of dedicated caregivers, ensuring they deliver exceptional care and support to clients.

• Oversee the clinical operations of private duty home care and licensed residential care homes, maintaining the highest standards of quality and safety.

• Collaborate with families and referral partners to create personalized care plans that meet the unique needs of each client.

• Foster a culture of accountability and consistency, encouraging team members to take ownership of their roles and responsibilities.

• Provide mentorship and guidance to caregivers, supporting their professional development and enhancing their clinical skills.

• Ensure compliance with all healthcare regulations and standards, keeping our practices aligned with industry best practices.

• Monitor and evaluate client care outcomes, using data-driven insights to continuously improve service delivery and client satisfaction.

Qualifications:

• Experience in leading and managing clinical teams, with a focus on fostering a culture of accountability and excellence.
• Ability to develop and implement personalized care plans, collaborating effectively with families and referral partners.
• Proven track record of maintaining compliance with healthcare regulations and industry standards.
• Strong clinical judgment and decision-making skills, with the ability to evaluate and improve client care outcomes.
• Excellent communication and interpersonal skills, enabling you to mentor and support caregivers in their professional development.
• Ability to inspire and motivate a team, ensuring the delivery of exceptional care and support to clients.
• Experience in overseeing clinical operations in settings such as hospitals, skilled nursing, home health, or hospice is preferred.
