Job Title: Physical Therapist – SNF
Location: Mauston, WI
Type: Full-Time/ Direct Hire | Variable Shifts
Salary: $66,000 – $102,000 annually

Position Overview:
Navitas Healthcare, LLC is seeking Physical Therapist – SNF for an exciting Travel or Local job in Mauston, WI.

Responsibilities

• Evaluate and treat patients experiencing mobility limitations, pain, weakness, or movement dysfunction
• Develop individualized treatment plans focused on restoring function and independence
• Assist patients in recovery following surgery, illness, or injury
• Help reduce fall risk and improve overall patient safety
• Support discharge planning and continuity of care
• Maintain accurate and timely clinical documentation including evaluations, daily notes, progress reports, and discharge summaries
• Coordinate with physicians, nursing staff, insurance providers, and rehabilitation team members
• Participate in maintaining a clean and safe treatment environment
• Perform additional duties as assigned

Qualifications

• Minimum 1 year of Physical Therapy experience preferred
• Graduate of an accredited Physical Therapy program
• Wisconsin PT license or ability to obtain upon acceptance
• Strong communication and patient-care skills
• Experience in Skilled Nursing Facility or rehabilitation settings preferred
