Social Services Director – Skilled Nursing Facility

Carmel Hills Wellness & Rehabilitation is seeking a compassionate and experienced Social Services Director to oversee psychosocial care in our skilled nursing facility. In this leadership role, you will support the emotional, social, and mental well-being of our residents, ensuring smooth transitions and a high quality of life.

• Bachelor’s degree in Social Work or a human services-related field (such as Psychology, Sociology, Special Education, or Rehabilitation Counseling).

• Minimum of one year of supervised social work experience in a healthcare setting working with elderly populations.

• Experience in a Skilled Nursing Facility (SNF) preferred.

• Complete psychosocial assessments and develop individualized care plans.

• Maintain accurate and timely documentation in compliance with state and federal guidelines.

• Provide emotional support and crisis intervention for residents and families.

• Coordinate discharge planning and referrals to community services.

• Advocate for residents’ rights and collaborate with the interdisciplinary care team.

• Educate staff and support compliance and survey readiness.
