Job Title: Registered Nurse Profession: RN Specialty: Case Management Duration: 13 weeks Shift: Day Shift Hours per Shift: 8:00 - 16:30 Experience: Minimum of 1 year of recent acute care discharge planning experience License: Active RN license required Certifications: None specified Must-Have: Recent acute care discharge planning experience Business casual dress code Description: This position involves working in case management. The unit has a total of 590 beds. Patient ratios may vary, typically ranging up to 1 nurse for every 25 patients. Staffing includes registered nurses, social workers, and certified medication aides. Documentation systems utilized include Meditech, Midas, and Wellsky. The schedule consists of Monday through Friday from 8:00 AM to 4:30 PM with rotating weekends. On-call requirements are not applicable for this role. Floating may occur to any unit as needed. Patient experience is a priority within the facility.
