ACT Nurse LPN Location: Metairie, Louisiana (Community-based) Employment Type: Part-time Salary: $48,000 - $51,000 Date Posted: 8/4/2026 About the Role Clarvida is seeking an ACT Nurse LPN to join our Assertive Community Treatment (ACT) Team serving adults with severe and persistent mental illness. In this role, you will be a critical part of a multidisciplinary team providing intensive, community-based services designed to support recovery, stability, and independence. ACT services are delivered where clients live, work, and spend time in the community. You will support a specialized caseload, helping clients achieve treatment plan goals while remaining integrated with family, friends, and community supports. This position includes use of a company vehicle for client transportation as needed and participation in a rotating on-call schedule, which provides additional earned PTO. Responsibilities Provide nursing services as part of the ACT multidisciplinary team Meet with clients in homes, community settings Conduct basic health assessments, including vitals and labs Support medication management and facilitate long-acting injectable medications Coordinate care with medical providers and psychiatric staff Support clients with severe and persistent mental illness and/or substance use disorders Participate in a rotating on-call schedule (approximately one week every 7-10 weeks) Provide client transportation using a company vehicle as needed Maintain accurate, timely, and compliant documentation Requirements Active Licensed Practical Nurse (LPN) license in the State of Louisiana Experience working with individuals with severe and persistent mental illness and/or substance use disorders Psychiatric nursing experience preferred Valid Driver's License in good standing and current automobile insurance Compensation & Benefits Full-Time Employees Competitive compensation based on experience Weekday schedule (8:00 AM-5:00 PM) with flexibility Paid vacation days that increase with tenure Separate sick leave that rolls over annually Up to 10 paid holidays* Medical, dental, and vision insurance plan options DailyPay - access earned wages without waiting for payday* Training, development, and continuing education credits for licensure requirements All Employees 401(k) Free counseling licensure supervision Employee Assistance Program Pet insurance Perks @ Clarvida - national discounts on shopping, travel, Verizon, and entertainment Mileage reimbursement* Company cell phone *Benefit availability may vary by position, state, or county.
