Job Description & Requirements Specialty: Physical Therapist Discipline: Therapy Start Date: ASAP Duration: 13 weeks 40 hours per week Shift: 8 hours, days Employment Type: Travel Job Title Physical Therapist Location Colorado Schedule / Job Type Contract, Days, Outpatient, Onsite Key Responsibilities Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction Conduct 60-minute initial evaluations and 30-minute follow-up sessions, progressing patients toward discharge within an established length of stay Complete medical record documentation in a clear, concise, accurate, and timely manner including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines Delegate and direct discipline-specific therapy services for assigned patients, ensuring functional treatment programs support effective return to community activities Serve as an educator to patients, families, students, and coworkers; participate in student programs by providing supervision, developing learning objectives, and completing student evaluations Adhere to the discipline-specific State Practice Act and all regulatory requirements, and actively pursue professional development through continuing education and professional organizations Required Qualifications Active and unrestricted Physical Therapist license in good standing in the state of Colorado Experience in outpatient physical therapy settings Proficiency in clinical documentation, goal writing, and compliance with state and federal guidelines Knowledge of and adherence to discipline-specific professional code of conduct and State Practice Act Completion of all state-mandated continuing education requirements Preferred Experience / Skills Experience supervising or mentoring physical therapy students in a clinical setting Familiarity with performance improvement methodologies and quality improvement initiatives Experience with safe patient handling practices and workplace safety protocols Why Join Us Join a patient- and safety-focused outpatient therapy environment that supports professional growth and individualized development planning Work within a structured, team-oriented setting that values quality care, accountability, and continuous improvement Concentric Healthcare Staffing Job ID 762538.
