Licensed Practical Nurse (LPN) -Topsfield, Massachusetts

About the Role:

As an LPN at our center, you will play a vital role in supporting residents, guiding care teams, and creating a safe, respectful, and compassionate environment every day. You will provide direct nursing care, supervise and support nursing assistants, and collaborate with the interdisciplinary care team to ensure excellent resident outcomes.

Responsibilities:

• Providing direct nursing care, treatments, and medication administration

• Supervising and supporting CNAs during daily care activities

• Monitoring resident conditions and reporting changes appropriately

• Assisting with admissions, transfers, and discharges

• Completing accurate and timely nursing documentation and incident reports

• Maintaining infection control, safety, and regulatory compliance standards

• Supporting a respectful, positive, and team-oriented environment for residents, families, and staff

• Following facility policies, nursing standards, and individualized care plans to ensure quality resident care

Requirements:

• Current, active Massachusetts LPN license

• Previous experience in long-term care, rehabilitation, or skilled nursing preferred

• Strong communication and teamwork skills

• Basic computer and documentation skills

• Ability to prioritize and make sound clinical decisions in a fast-paced environment

• Ability to communicate effectively with residents, families, physicians, and staff
