RN - Care Coordinator Profession: RN Specialty: Critical Need Duration: 13 weeks Shift: 8 hours Hours per Shift: 08:00 AM to 05:00 PM Experience: Minimum 1 year of case manager or Patient Care Coordinator experience License: Active RN license (Oregon & WA RN or compact license required) Certifications: BLS Must-Have: 1 year of case manager or Patient Care Coordinator experience Ability to work weekends (Sunday-Saturday) and holidays Previous experience with EPIC charting Experience with Medicaid/Medicare Description:Seeking a skilled RN to serve as a Patient Care Coordinator in a critical need capacity.The candidate must possess an active RN license in Oregon or Washington, or a compact license.Candidates should have at least one year of experience in case management or as a Patient Care Coordinator.The successful candidate will be required to work weekends and holidays.Previous experience with EPIC charting is preferred, as is familiarity with Medicaid and Medicare processes.A commitment to providing exceptional patient care and coordination is essential. #J-18808-Ljbffr Cynet HealthStaff
