Responsibilities
- Prescribes, coordinates, and delivers nursing care using the nursing process; supervise CNAs and coordinate care with other health care workers.
- Conduct comprehensive initial assessments and ongoing reassessments of physical, psychosocial, environmental factors; adapt Plan of Care as needed and establish discharge plans with patient, physician, other disciplines, and caregivers.
- Complete OASIS data and start-of-care activities; ensure care is provided in compliance with Federal, State, and Agency regulations.
- Assess educational needs of patients, families, and caregivers; implement appropriate teaching strategies and document outcomes.
- Maintain accurate documentation per regulatory guidelines; promote privacy and confidentiality; uphold professional standards of conduct.
- Promote a clean, safe environment of care and participate in care coordination during transitions of care as appropriate.

Requirements
- Active IL RN license (required; copy included in submission)
- Home Health experience (required)
- Active BLS or willing to obtain (required)
- Valid driver's license and auto insurance (required)
- COVID-19 and influenza vaccination required; vaccination status to be provided in profile (declinations not approved)
- OASIS and start-of-care experience strongly preferred
- Resume and a Skills checklist (within 1 year) required
- Two supervisory references required
- Certifications (active) and IL nursing license (active) must be provided with submission
- COVID card/status (documentation required)
