Overview
Physical Therapy Assistant, LTAC in Hampton, NH. Start date: 08/17/2026. Duration: 13 weeks. Weekly pay: 1,490–1,528 per week. Guaranteed hours: 35 per week. Shift: 5x8 Days. Setting: LTC - Assisted Living Facility. This travel assignment is managed by Healthforce Healthcare LLC.

Responsibilities
- Provide physical therapy assistant services under supervising physical therapist in a LTAC/long-term care setting
- Conduct patient evaluations and document progress, including range of motion, strength, balance, and functional mobility
- Implement and progress treatment plans with goals such as gait training, therapeutic exercises, balance training, and activities of daily living (ADL) practice
- Administer modalities and therapeutic interventions as directed, ensuring patient safety and comfort
- Maintain accurate documentation of treatments, responses, and goals; communicate changes to the supervising PT and rehab team
- Ensure proper use of equipment, infection control, patient positioning, and adherence to safety protocols

Requirements
- Active Physical Therapy Assistant license in New Hampshire or eligibility for licensure
- Associate degree in Physical Therapy Assistant from an accredited program
- 1 year of PT assistant experience in a rehab, LTAC, or skilled nursing setting preferred
- Ability to work effectively in a long-term care/assisted living environment
- Strong communication and documentation skills
