Enhanced Adult Mental Health Services (EAMHS) provides services to individuals with an SPMI diagnosis in an outpatient setting and in the community as needed. We work from a team based model to support our clients with a variety of services to help build skills for stability and independence. The Community Case Manager assists with theimplementation of treatment plans, provision of individual and group skillstraining, case management, and coordination of multiple service providers forclient caseload with other care team members. Provides services primarily inthe community, but also onsite. Keeps accurate and timely records ofactivities. Location: This position is based out of LifeWorks NW's King site. Pay/Benefits: $25.39-$29.05 per hour, plusbenefits Salary Placement Notice: The posted salary range reflects the compensation potential for this role. Most candidates will be placed between the entry level and midpoint of the range, depending on their experience and qualifications. However, more seasoned professionals with extensive exact or similar experience may be considered for placement toward the higher end of the range. Final salary offers are made in alignment with internal compensation processes in accordance with Oregon Pay Equity laws. Responsible forclient orientation including advising clients and significant other(s) ofthe services offered through the program and recommend services. Provides inputon clinical assessments and the development of treatment plans. Providescase-coordination by maintaining contact with other agencies, referralsource(s) and staff regarding client and significant other(s) involvementin treatment services. Conductsindividual and group skills trainings sessions based on current evidencedbased practices e.g. collaborative problem solving, cognitive behavioralinterventions, and solution focused therapy. Teachessocially acceptable behavior, employing techniques such as behaviormodification and positive reinforcement. Encourages individuals and familymembers to develop and use skills and strategies for confronting theirproblems in a constructive manner. Observes andinfluences clients' behavior, communication and interaction. Detectsunusual behavior and reports observations to professional staff. Organizes,supervises, and encourages clients to participate in social, educational,and recreational activities. Provides assistance and support to clientswith the job search process. Assists theclient to the extent necessary, based on client's level of functioning, toaccess needed services and resources. Advocates on client's behalf.May attend the client's psychiatric medical appointments, if clinicallyindicated. Reviews andevaluates clients' progress in relation to measurable goals described intreatment plans. Providesclients or family members with information about mental health andaddictions issues and about available services and programs, makingappropriate referrals when necessary. Reports anypertinent information to the proper authorities in cases of endangerment,neglect, or abuse of children, individuals with severe and persistentmental illness or the elderly. Acts as aclient advocate to coordinate required services or to resolve emergencyproblems in crisis situations. Provides clinical support to clients withimmediate and urgent needs Providesassistance with team tasks (taking responsibility in one particular areai.e. arranging transportation). Maintains chartnotes related to external contracts, obtains pertinent records from othertreatment providers, and tracks client's involvement, follow-through andprogress in varied treatment activities. Completes documentation inelectronic health record system in a timely manner. Checkse-mail/voice mail daily/throughout the day when working responding tocontacts within 48 hours for routine matters (urgent matters mayrequire a more immediate response). Transportsclients in personal or agency vehicles and travels between sites or in thecommunity. Qualifications: This positionrequires a valid and active QMHA credential certification through MHACBO. Bachelor'sdegree in Psychology, Counseling, Social Work or a mental health relatedfield OR a minimum of 3 years of prior experience with the relatedpopulation in the counseling field. Demonstratedability to communicate effectively; understand mental health assessment,treatment and service terminology and individual, group, family and othercounseling techniques; program policies and procedures for services andsupports identified in an Individual Services and Supports Plan. Demonstratedability to provide culturally-competent treatment services to diverseclient populations and maintain a cooperative working relationship withothers in a culturally diverse environment. Demonstratedability to provide psychosocial skills training and implement elements ofa treatment plan. Demonstratedability to provide case management services for people with severe andpersistent mental illness in compliance with confidentially policies. Knowledge ofcommunity resources and treatment agencies enabling appropriate referralsfor specific client needs. Proficient inMicrosoft Outlook and Word. Ability to master proprietary softwareincluding electronic health record system and scheduling system required.
