Responsibilities:

• Provide social work services to patients and their families

• Develop and implement individualized treatment plans to address patients' social, emotional, and behavioral needs

• Collaborate with interdisciplinary teams to ensure comprehensive care and address patients' concerns

• Conduct assessments and screenings to identify patients' social and emotional needs

• Provide crisis intervention and behavioral services as needed

• Facilitate patient education and support groups

• Maintain accurate records and documentation of patient services
