Position Summary The Discharge Coordinator is a professional who plans, manages and coordinates the discharge plan of the acutely hospitalized patient. The Coordinator works in conjunction with the multidisciplinary team and collaborates with appropriate team members along the continuum of care. The Coordinator is knowledgeable of available community resources and the healthcare benefits of the acute patient. The Coordinator will have excellent communication skills both written and verbal and be able to work in an intense environment. Required Qualifications Previous acute care, healthcare, or related experience. Preferred Qualifications One year or above acute care hospital discharge planning experience. Working knowledge of regulatory guidelines including Medicare/CMS guidelines, TJC standards, and NCQA standards. CA RN or LVN license. Physical Requirements Must have dexterity, coordination, visual and auditory acuity to perform all jobs. Can sit or walk for long periods of time.
