Northern Light Eastern Maine Medical Center Bangor, ME Department: Structured Heart Position is located: Northern Light Eastern Maine Medical Center Work Type: Full Time Hours Per Week: 40.00 Work Schedule: 7:00 AM to 5:30 PM * Sign On Bonus Available* Summary: Provide nursing care in physician practices, duties can vary by practice specialty. Carry out instructions provided by clinical coordinator(s), RN Practice Manager, clinical supervisors and Director, Clinical Services of Physician Practices. Responsibilities: * Patient navigation through scheduled pre-operative testing; post-operative care at home * All Structural Heart Program registries management * Referral Management * Patient triage calls * Structural Heart statistics * Communicate effectively in both oral and written form, as appropriate for the needs of the audience. Assist departments with questions. * Provides excellent service with focus on patient and/or customer experience and ensures all functions related to customer service supports the department, member organization, and patient related goals. * Assists with staff education and training as appropriate. * Performs administrative tasks associated with the operations of department and/or organization. * Follows established procedures and standards to limit the spread of infection. * Directly leads work efforts of subordinates and/or peers. * Works to meet patient's psychological, emotional, or physical needs. * Shares knowledge and serves as resource in a team environment. * Performs other duties as assigned or required.
