Galen is CHATTANOOGA'S Doctor!Summary/Objective:The Patient Engagement Coordinator (PEC) is a member of the Galen Value-Based Care team responsible for promoting patient engagement specific to quality care metrics to improve or maintain the patient’s social, emotional, functional, and physical health status. The role of the PEC is patient outreach to establish or maintain routine contact with his/her primary care physician and facilitate specialist referrals as appropriate per physicians’ orders, with an emphasis on annual wellness visits and preventative screenings, and other clinical quality measures under the direction of the Population Health Manager.The Patient Engagement Coordinator enhances the partnership between the patient, Galen care providers and the patient’s insurance provider by proactively connecting the patient to care management programs offered through their insurance, as well as other community resources. This position reports to the Population Health Manager. It is the obligation of each Galen associate to comply with and promote the Galen mission, core values, privacy, and corporate/departmental/site policies (I.E. Dress code, Time and attendance, et al).Essential Functions:Patient Outreach for Annual Wellness Visits and other VBC initiativesPatient Roster ManagementMonitoring and tracking outreach attempts in the Electronic Health Record (EHR)Duties/Responsibilities:Reconciles payer rosters for patients in Value-Based Care panels, with emphasis on non-engaged and new members.Provides telephonic outreach to patients under Value-Based Care panels to encourage engagement with PCP and adherence/compliance to other clinical quality measures.Maintains engagement with patients to ensure scheduled services are completed.Completes assigned outreach and follow up in a timely manner, as directed by management.Maintains professional rapport with clinicians, support staff, and other Galen team members.Participates in team, payer, or department meetings as needed and directed by management.Respects patients by recognizing their rights; maintaining confidentiality.Protects self and other team members by following all compliance, HIPAA and infection prevention guidelines and regulations.Actively and consistently supports the mission, vision, and values of Galen Medical Group by providing excellent care delivered with wisdom, compassion, integrity, and a commitment to technology, education, and scientific inquiry.Knowledge/Skills/Abilities:Excellent customer service skills.Exceptional skills of verbal and written communication, problem solving, professional interaction.Self-reliant with the ability to work independently, and take initiative to set and meet goals.Must be detail oriented and well organized.Must be PC literate with strong computer navigational skills and knowledge of Microsoft Office programs such as Excel, Office, etc.Strong time management skills, and ability to work within specified timeframes and deadlines.Knowledge of basic medical terminology is helpful.Qualifications:Education:High School Diploma or equivalent is required.Please note this job description is not designed to cover or contain a comprehensive listing of activities, duties, or responsibilities required of the employee for this job.
