Minimum Qualifications:

• High school diploma or equivalent.
• Certification in CPR and First Aid.
• Experience working with individuals requiring personal care or support, preferably in a healthcare or social services setting.
• Ability to pass background checks and comply with state and federal regulations related to caregiving.
• Strong communication skills and a compassionate, patient-centered approach.

Preferred Qualifications:

• Post-secondary education or training in healthcare, social work, or a related field.
• Certification as a Certified Nursing Assistant (CNA) or equivalent.
• Experience working with individuals with developmental disabilities, mental health conditions, or chronic illnesses.
• Knowledge of person-centered planning and trauma-informed care practices.
• Bilingual abilities or experience working with diverse cultural populations.

Responsibilities:

• Provide direct, hands-on care and support to individuals in accordance with their personalized care plans.
• Monitor and document changes in health status, behavior, and overall well-being, reporting concerns to appropriate healthcare providers.
• Assist with daily living activities such as hygiene, mobility, meal preparation, medication reminders, and transportation.
• Collaborate with multidisciplinary teams to update care plans and ensure coordinated, holistic care delivery.
• Promote social engagement and community integration by facilitating participation in recreational and therapeutic activities.
