Physical Therapist (PT)

Role Overview: The Physical Therapist provides direct rehabilitation care in a skilled nursing environment, supporting patients with long‑term and short‑term post‑acute needs. Key Responsibilities: Develop and implement individualized treatment plans based on physician referrals. Assess, modify, and track patient progress towards functional goals. Supervise and evaluate care provided by PTAs, rehab aides, and students. Maintain accurate documentation and utilize therapy software effectively. Collaborate with interdisciplinary teams, communicate with patients and families, and ensure patient safety and rights. Follow all laws, regulations, and company policies, including confidentiality and safety standards. Participate in community health initiatives when appropriate. Qualifications & Skills: Graduate from an accredited physical therapy program (MSPT or DPT preferred). Active licensure in the applicable state; CPR certification required. Excellent communication, organization, and interpersonal skills. Proficient in Microsoft Office and therapy documentation tools. Ability to make independent decisions, demonstrate good body mechanics, and work effectively within a team. Preferred one year of experience; new graduates are welcome to apply.
