Description

• Conduct thorough evaluations of patients in both inpatient and outpatient environments, tailoring treatment plans to meet individual needs.
• Apply various physical therapy techniques aimed at enhancing mobility, building strength, improving balance, and restoring function.
• Collaborate closely with a multidisciplinary team including nurses, physicians, case managers, and fellow rehabilitation professionals to maximize patient outcomes.
• Guide patients and their families through therapeutic objectives, discharge planning, and home exercises.
• Keep meticulous records of evaluations, treatments, and patient progress in compliance with facility and regulatory standards.
• Engage in care team rounds and interdisciplinary meetings to ensure cohesive, coordinated care for every patient.

Education

• Doctorate in Physical Therapy (DPT) or a comparable degree from an accredited institution.

Certifications

• Active state licensure as a Physical Therapist.
• CPR certification preferred.
• Additional specialized credentials (e.g., neurological, geriatric) are advantageous, although not mandatory.

Skills

• Previous experience in a clinical setting is highly preferred.
