Transplant CoordinatorSentara Healthcare is seeking a Transplant Coordinator for our Kidney/Pancreas Transplant team at Sentara Norfolk General Hospital.Shift: Days M-FOutpatient clinic is open Monday – Friday between 8am and 4:30pm. Transplant Coordinator will have some scheduled call and have weekend call about every 11th week.Position Highlights:Innovation: 525-bed Level 1 trauma center and tertiary care facility that was first in the region to perform kidney, heart, and pancreas transplants.Market: Sentara Transplant Center has been around for nearly 40 years and is the only transplant center in Hampton Roads Region. We have performed more than 2,200 transplants.Nursing Excellence: We were the first hospital in the region to become a Magnet® recognized hospital.What you will do:Coordinate care with post-transplant patients.Facilitates clinic flow, transplant evaluation, and patient care management.Provide weekly education sessions to patients and support member.
