Under the order of the physician, the (PT) assumes primary responsibility for providing skilled physical therapy and health teaching to patients and their significant others/caregivers in the home setting. Responsibilities:

1. Conduct physical assessment/histories to determine health problems, homebound status and follow plan of care/treatment plan in collaboration with the physician.
2. Observe patient's condition to monitor response to treatment and communicate/document changes to physician.
3. Counsel and teach home program to patient and significant other /caregiver to accomplish goals.

1. Collaborate with scheduler on a regular basis for coordination of patient care/schedules.
2. Maintain clinical EMR and paper records according to agency, state and federal guidelines.
3. Develop home health aide assignments and provide supervision/guidance on physical therapy only cases.
4. Attend case conferences/team meetings as scheduled.
5. Attend skills-fair and other mandatory educational programs.
6. Provide supervision to licensed physical therapist assistants.

QUALIFICATIONS To perform this job successfully, an individual must: Possess current PT license in the State of Florida Have a minimum of 1year experience as a PT *This position requires screening through the Care Provider Background Screening Clearinghouse.
