This role, based remotely, focuses on addressing the complex emotional and mental health needs of individuals nearing the end of life, their families, and caregivers. The successful candidate will employ their clinical skills to enhance the quality of life for clients by managing symptoms and facilitating end-of-life discussions.

Work Environment\n

This full-time position allows for a flexible remote work environment, with the opportunity to engage directly with clients virtually. Our care team prioritizes collaboration, making it easy to reach out to fellow professionals during challenging situations to ensure coordinated and effective support for our clients. Standard work hours are Monday to Friday, from 8:00 AM to 4:30 PM, with occasional evening or weekend availability as needed based on client requirements. Key Responsibilities\n

  • Conduct thorough assessments of assigned clients to determine their psycho-social needs, providing individualized treatment plans that reflect best practices in hospice care.

  • Develop and implement therapeutic interventions that include crisis management, emotional support, and effective coping strategies tailored to the client’s unique circumstances.

  • Collaborate closely with interdisciplinary teams, including physicians, nurses, and chaplains, to ensure holistic care approaches that improve overall patient outcomes.

  • Provide support and counseling to families, helping them navigate the emotional challenges associated with end-of-life care and decision-making.

  • Utilize electronic health record (EHR) systems to maintain accurate documentation of client interactions, treatment progress, and necessary follow-ups in compliance with organizational policies.

  • Participate in regular team meetings and case discussions to share insights, strategize, and enhance collaborative treatment approaches, ensuring alignment with overall mission and objectives.

Patient Population / Client Focus\n

The primary focus of this position is to support clients facing terminal illnesses and their families. Previous experience or familiarity with the emotional, legal, and clinical considerations surrounding end-of-life issues is essential. The clinician will play a critical role in guiding conversations about advanced directives, care preferences, and grief processing, contributing significantly to clients' emotional well-being and quality of life.

  • The ideal candidate must possess a Bachelor’s Degree in Social Work (BSW) with the requisite state licenses, including an LCSW designation.

  • This position offers a competitive annual salary ranging from USD 85,000 to 90,000, commensurate with experience.
