Registered Nurse Case Manager

$2040.00/week - Job Summary: Provide specialized case management nursing services within a hospital setting to facilitate patient care coordination, discharge planning, and resource management. Ensure optimal patient outcomes through assessment, planning, and collaboration with multidisciplinary teams. Responsibilities: Assess patients’ clinical needs and develop individualized care plans. Coordinate patient services and facilitate communication among healthcare providers. Monitor patient progress and adjust care strategies accordingly. Educate patients and families on treatment plans and post-discharge instructions. - Document all patient interactions and care activities accurately using electronic health record systems. Collaborate with medical staff and social workers to ensure timely discharges and resource utilization. Qualifications: Valid RN license in the state of practice. Minimum of 2 years’ experience in an acute care or hospital environment. Strong knowledge of case management principles and hospital workflows. - Excellent communication and interpersonal skills. - Ability to multitask efficiently in a fast-paced setting. - BLS/ACLS certification preferred.
